"Wrong username or password" for everyone including admin after logging out

We are noticing this problem. Once users log out, they are unable to log back in. After attempting to type the password after 5 times, we all get the 15 minutes lockout message.
We have tried logging in as different users, and even tried password retrieval/change, and still unable to log in.
We registered a new user, then used the tools.php to change access to admin. But once logged out, cannot log in (for that new user)
Everyone else seems to be locked out when they log out.
Running 3.7 gold. Need help getting this fixed, thanks

Also, we created a user called "test" and password is "password". Can you tell us if you are able to log in and out that user? click HERE (http://www.ravelinks.com/forums/login.php) to try logging in

I can't think of any possible explanations unless there is a customization causing problems. You can determine this by temporarily disabling your customizations. Try to reproduce it on a default style:
Admin CP -> Styles & Templates -> Style Manager -> [Add New Style]
Create a new style with no parent. Then click that style's name in the Style Manager to view your forum with that style.
If you still have the problem then try disabling your plugins:
Admin CP -> vBulletin Options -> Plugin/Hook System
Also run a file check to look for customized files:
Admin CP -> Maintenance -> Diagnostics -> Suspect File Versions
